My name is Dr. Stephen Clark and I have been a Doctor of Physical Therapy for 30 years and now I’m in my early 50s. Over my career, I’ve treated World Class Olympic and professional athletes as well as Hollywood’s highest paid musicians, actors and producers.

 

But more importantly, I’ve treated tens of thousands of patients just like you. My enormous amount of experience has led me to a simple truth about middle-aged men with low back pain and I want to share it with you in my workshop-for-men or “Manshop”.

My unique approach to treating men with low back pain goes beyond common and becomes revolutionary. Revolutionary sounds to good to be true but what if I could help you feel and move like you did 10 years ago? Doesn’t that sound like a revolution?

When I was younger, I couldn’t image getting low back pain. I felt great. I could bend, lift and twits against any force and play any sport with no ill consequences. Symptoms like soreness, stiffness, pain, aches, burning in my low back, hips and legs were non-existent.

But slowly and gradually, as I aged, I began to realize I’m starting to feel like all of my patients. Wait a minute! This can’t be happening to me. Was I going to end up like all of my patients looking and feeling older to early with low back pain?

No way. I put my heart and sole into figuring out how to stop low back pain and I’ve been so successful that I must share this in my “Manshop” with other men while we still have time.

George (not his real name) was just feeling old and tired. But he was only 46 years old. Sure he had 2 younger kids that kept him busy on the weekends. He had a majority sedentary job that went beyond 9-5pm and often times had to beat early morning traffic and come home hungry.

George knows he’d been putting off his low back pain but can’t remember when it really started. He had years of on-and-off low back pain that was never quite serious enough to go to the doctor. But now it’s getting worse. And more frequent. And lasting longer.

“My low back and hips have been locking up lately and I had to miss some of my basketball league games,” he confided.  “All the driving back and forth to work, sitting at a desk all day has made me so stiff I make noises when I get off the couch!”

George started doing some home stretching and light weightlifting at home to see if that helped but he made no progress. His low back pain worsened and became almost constant. So he stopped doing any exercise because he wasn’t sure what to do.

“As I became more sedentary I started to gain a few extra pounds which was completely depressing because  I already had 30 pounds to loose.”

So what do men do when they have a problem? They ask their other guy friends that’s what. George went to his friends, explained the low back pain and sure enough, his friends were nodding their heads.

“I know exactly what you’re talking about George.  I get that back pain all the time. My wife has been telling me to loose weight but I cant exercise enough to break a sweat.  I went to a physician and got x-rays and an MRI for my back pain. They also prescribed medications but nothing worked – now they say I might need surgery?”

The consensus among his friends was that medication and surgery were not an option. Who has time for back surgery and what if it doesn’t work? No one likes popping pills and we all know that this doesn’t fix the problem it just covers it up until it gets worse.

“Hey George,” said his friend still playing on his basketball team. “I went to Athletic Physical Therapy and graduated from their Low Back Pain Bootcamp and in 6 weeks eliminated my low bak pain. You should call them.”

George was asked to do some typical physical therapy movements like standing back bends and touching his toes. These were all done normally and it did not increase his pain. Now you may be thinking to yourself. How could a physical therapists with all of my experience call a normal range of motion test on the low back “typical”? Aren’t I kind of slamming all the doctors who ALL do the same old test?

Well in fact you’re correct. The basic low back or lumbar spine assessment that is commonly done on all patients by all doctors is exactly that = COMMON! It is purely a screening tool used to see which direction a lumbar assessment should be directed. If these basic motions increase pain from moderate to severe then physical therapy is indicated.

But what if a patient has mild nagging middle aged low back pain and they are trying to fix it BEFORE it becomes worse? How do you test for that?

Here is how you do it and this is where it got complicated for poor George.

George was asked to do 10 coordinated movements in a number of different positions. Each test was designed to measure his strength and flexibility of his low back that would simulate the stresses that are required to protect and prevent the low back from hurting.

Each movement was carefully rated by me to assess if each part of George’s body was moving in the proper manner with regard to joint mobility, coordination, strength and balance. He was given a score of 0 = unable, 1= able or 3 = exceeds for each of the 10 different movements.

When I added up his score he got a total of only 9 out of 30 possible points.

The score is used as a predictor for low back pain:

WORSENING LOW BACK CONDITION SCORE

<10 points = probable 11-20 points = potential >21 points = unlikely

I have to admit these movements to detect low back pain and prevent low back injury are not easy to do. But you’re forgetting something. You could do these exercises in your SLEEP when you were 30 years old. They are hard because you are starting to get old and you are not doing the necessary movements to keep yourself strong, flexible and coordinated. Your are driving, sitting twice as much and taking care of your body less.
